Whenever American Action Forum President Doug Holtz-Eakin testifies before Congressional Committees about healthcare spending, he always emphasizes the need to first create a budget. If the US had a set healthcare budget, policymakers could work within those limits to get the most value possible for our healthcare dollars. Unfortunately, because Medicaid and Medicare are unlimited entitlement programs, and with the Affordable Care Act (ACA) creating a new entitlement program of health insurance subsidies, the limit to healthcare spending is out of sight.

The Urban Institute recently released a paper on the decline of health care access in the United States for both insured and uninsured adults. The study measured access with three criteria: the likelihood of having unmet medical needs due to cost, receiving a routine checkup, and receiving a dental visit; looking at all nonelderly adults and the subgroup of uninsured adults.

Last week, New Jersey Governor Chris Christie vetoed the state legislature’s bill to create a state health insurance exchange, as mandated by the Affordable Care Act (ACA). Governors who oppose the ACA are in a tough position regarding the creation and implementation of state exchanges.

Rewarding quality health plans is an admirable goal for the Medicare Advantage program. Unfortunately, the current system of linking star ratings to bonus payments and rebate adjustments instituted by the Patient Protection and Affordable Care Act (and expanded by the CMS Quality Bonus Payment Demonstration) fails to achieve that goal, and depending on its specific implementation, may even be counterproductive.
